Since childhood, the patient felt that their mouth was very protruding. Although the front teeth looked relatively straight, they still appeared buck-toothed when smiling. Meanwhile, the bite felt very uncomfortable on one side, brushing teeth always caused bleeding, and there was bad breath. Looking from the profile, the mouth appeared bulging and tense. Therefore, the patient came to the clinic seeking orthodontic treatment.
 

Treatment Plan:

The doctor analyzed the patient's intraoral and facial aesthetics, performing aesthetic analyses of the smile and big smile from both the profile and frontal views, and concluded: First, the patient's facial profile was protruding, making extraction orthodontic treatment necessary for significant improvement. The tension in the patient's chin muscle was also caused by the excessive protrusion of the lower front teeth. Intraoral examination revealed an ectopic tooth in the upper jaw that had no functional occlusion with the lower teeth, leading to long-term unilateral chewing and subsequently causing facial asymmetry. Treatment plan: Using fixed orthodontics, extract four first premolars, retract the protruding teeth while aligning them, establish an anterior smile line, and guide the patient's mandibular occlusal plane to rotate counterclockwise.
 

Treatment Outcome:

From the facial profile, it can be seen that the patient's profile protrusion has been perfectly resolved, transforming from a convex profile to a perfect straight profile. Meanwhile, the chin muscle has relaxed, the perioral muscles have improved, and the face looks younger. Intraoral observation shows that the patient's teeth are neatly aligned, the bite is tight, and the dental arch is ovoid.
